Referral Guide for Proactive

GPs on Medtech and indici can now refer to Proactive clinics nationwide on SR.
Your patients will be supported by an integrated team of health professionals with the experience and expertise to meet their needs.
Proactive clinics have been grouped by region, making it easy for you to find local clinics.
Simply open SR, search Proactive, choose the clinic and complete the referral.

Search On the main Search type ‘Proactive’ and choose Proactive regional entry from the dropdown list.

Select On the Profile page for Proactive click on the ‘e-Refer’ button and select the clinic.

Complete the SR referral form in Medtech or Indici as usual…

Patients will have access to:
Physiotherapy, Occupational Therapy
Clinical and Neuro Psychology,
Occupational and Environmental Physicians,
Clinical Exercise Physiologists,
Occupational Health Nursing,
Speech and Language Therapy,
Social Work & Dietetics
