Purpose and use of information we observe: The statistical information referred to above will be viewable by website administrators and certain other Proactive and Evolution Healthcare staff whose role it is to view this data. The purpose of collecting and analysing this information is to personalise our Site, provide customised content and information, monitor, and analyse the effectiveness of the Site and marketing activities, monitor Site usage, and track entries, submissions, and status in any promotions or other activities on the Service.
Disclosure of information we observe: Statistical information referred to above may be shared with, and held by, third-party contractors to the extent necessary for those contractors to provide web services, such as website hosting, improvement, and maintenance.
You may voluntarily provide us with information in various ways on the Sites. For example, when you register for an account, make an on-line booking, use our portal, online forms, post information, interact with other Users through communication or messaging capabilities, or send us customer service request or email. This information will be held by Proactive in accordance with the Privacy Act 2020 and the Health Information Privacy Code 2020, which regulates how we can collect, use, hold, disclose, access, correct, manage, and dispose of your personal information.
Disclosure of information you have given us: We will not disclose your personal information for purposes that you did not give it to us for. If you have provided a request or feedback via email or online form, that request/feedback with your information will be sent to appropriate Proactive staff to respond. We may pass relevant comments on to other people within Proactive who need to assist with your request/feedback or, if relevant, who administer or contribute content to our Sites. We will not disclose your personal information to any third-party without your consent, unless required by New Zealand law.
We work with third party service providers who provide website, application development, hosting, maintenance, and other services for us. These third parties may have access to, or process information as part of providing those services for us. We limit the information provided to these service providers to that which is reasonably necessary for them to perform their functions, and our contracts with them require them to maintain the confidentiality of such information.
