You do not need a referral to attend physiotherapy and we are able to lodge an ACC Claim for you if you meet ACC criteria. You are also able to access private or other insurance based physiotherapy if you do not meet ACC criteria.
To refer to the Wellington Concussion service – for ACC to fund the referral the client needs to be between 10 days – 12 months post injury, have an accepted ACC claim and be referred by their GP, Accident and Medical Centre or ACC Case Manager.
To send a referral for the concussion service, pain management, community rehabilitation, housing assessment, home assessment, wheelchair assessment, return to work service and pre-post operative rehab, and any of our other services listed on our services page, please refer to referrals@proactive4health.co.nz.
Monday – Friday: 8:00 am – 5:00 pm There is parking available within the parking area surrounding the Wakefield Specialist Centre as well as parking up a ramp at the end of the carpark area for further parking options. There is also street parking just outside the main entrance. You are welcome to enter through the main door of the Wakefield Specialist Centre that is opposite the Rintoul street entrance to Wakefield Hospital and access the lift to Level 4 or enter through the main door to our entrance and up 1 set of stairs to our floor and main reception entry.
