Every injury is different, every person recovering from an injury has different needs. At Proactive, our mantra is to look after the whole person, not just treat the injury. The first step in providing a service for the whole person is to get a comprehensive understanding of their current situation. The Social Rehabilitation Needs Assessment is a service aimed to do just that. With this service, we are able to meet with you to get a holistic understanding of your injury and how it affects your ability care for yourself, to cope at home, your interactions with whanau and your ability to engage socially. The service will ensure that it considers a wide range of factors that may contribute to your current situation, including medical or injury history, social and cultural needs, and anything else that is important to you.
This will give us a picture of your overall rehabilitation needs, and with your input our clinical assessors will be able to make recommendations that will help you to recover better and will improve your overall health, wellbeing and quality of life.
These recommendations may lead to the provision of equipment to help improve your ability to get back into meaningful activities, it may lead into a rehabilitation programme or home help and personal care support.
