To refer to the Wellington Concussion service – for ACC to fund the referral the client needs to be between 10 days – 12 months post injury, have an accepted ACC claim and be referred by their GP, Accident and Medical Centre or ACC Case Manager.
To send a referral for the concussion service, pain management, community rehabilitation, housing assessment, home assessment, wheelchair assessment, return to work service and pre-post operative rehab, and any of our other services listed on our services page, please refer to email@example.com.
